Certificates of Insurance

State Agencies are often requested to furnish proof of general liability, auto, or property insurance in contracts or leases in order to satisfy contractors or other parties. Managers, supervisors, and employees ultimately have responsibility for procuring a Certificate of Insurance when it is required by law or by contract.

Certificates of Insurance for Auto Liability

Most University-owned vehicles typically contain a Certificate of Insurance for Auto Liability in the glove box. If your vehicle does not have one, it may be downloaded from the Risk Management and Tort Defense Division’s website: http://mt.gov/doa/rmtd/insurance/files/coi_al.pdf

University supervisors, managers, and employees are ultimately responsible to assure that the Certificate of Insurance is in the University owned/leased/loaned vehicle before it is driven.

Certificates of Insurance for Other

To request a Certificate of Insurance for a University supported activity, university insured vehicle or university property please fill out the attached certificate of insurance request form. You will find the Certificate of Insurance Request Form on RTMD’s website at http://rmtd.mt.gov/insurance/certofins1.asp. Completed forms should be returned in campus, emailed, or faxed to Kathy Benson, Environmental Health and Risk Management (bensonkm@mso.umt.edu , fax 243-6452). Please allow 5 business days for processing. Beginning July 1, 2010, all proof of insurance requests must be submitted on the Certificate of Insurance Request Form. If you have any questions don’t hesitate to contact me at 243-2700. Certificates of Insurance are not available for activities or those entities that the Risk Management and Tort Defense Division does not insure. Example: State employees using personal vehicles must carry evidence or proof of their own personal vehicle insurance since that insurance coverage is primary.

To obtain the desired Certificate of Insurance you will need to provide the following information:

  1. Name of the Requesting Agency, Division or Department
  2. A description of the event/activities/property being insured.
  3. Effective dates of the activity or the needed coverage.
  4. Name, Address, City, State and Zip code of the Certificate Holder (individual requiring certificate)
  5. The University is self insured by the State and pursuant to §2-9-101, MCA through §2-9-305, MCA the State of Montana is unable to name third party contractors as additional insured.
